Disease State Management

The Obesity Disease State Management Program is available to all PHCP members who possess a Body Mass Index (BMI) of 30 or greater.  BMI is a measure of your weight in relation to your height.   To calculate BMI, divide your weight (in pounds) by the square of your height (in inches) and then multiply by 703.   “Obese” is indicated by a BMI of 30 or greater.  As a participant in the program, you will receive monthly calls by our Registered Nurse Case Manager to offer support and guidance as you work toward your weight loss goals. 

In addition, participants in our Obesity Disease State Management Program may take advantage of the following benefits:

- Reimbursed membership in the Center for Fitness and Health including monthly fitness training and nutritionist consults;
- The Dr. Dean Ornish Program; and
- “Choose to Loose” classes at Monongahela Valley Hospital.

The Diabetes Disease State Management Program is also available to any member of PHCP with Diabetes.   As a participant in our program, you will receive calls from a Registered Nurse Case Manager to identify and evaluate your diabetes management needs and assist you in obtaining services related to your diabetes care.   As part of your participation in the program, the Diabetes Education Courses provided at Monongahela Valley Hospital will be offered to you as a covered benefit.  If you would like more information about our Diabetes Disease State Management Program, or to see if you qualify, please call our Utilization Review Department at 724-379-4011, Option 2.
